governed by the four Lutheran Churches of Casper, Wyoming. |

| Types of Facilities: | ||
| Shepherd of the Valley Care Center is a 24-hour skilled nursing care
facility. Maurice Griffith Manor is an board and care facility and located on the same site. |

| Services Available at Shepherd of the Valley Care Center: | ||
| Rehabilitation Services: | ||
| Physical therapy, occupational therapy and speech therapy are offered in-house by licensed specialists. | ||
| Medicare & Medicaid Certified: | ||
| All beds in the facility are certified for both Medicare and Medicaid. This means that you or your family member can remain in the same room as your nursing skill needs change. | ||
| Secure Unit: | ||
| The facility has a secure unit for those who wander and need extra security. There is a private dining room located in the area and a secure yard to allow the resident to be outside when the weather permits. | ||
| Church on-site: | ||
| Good Shepherd Lutheran Church is on the same site and a climate controlled access is available to those residents wishing to attend church. | ||
| Full Time Chaplain: | ||
| Reverend Marvin Skogen is the in-house Chaplain. | ||
| Specialized Professionsals | ||
| Medical Director - Dr. Martin Ellbogen Sr. is our in-house Medical Director. Psychotherapist - Dr. H. E. Chesteen is our in-house Psychotherapist. Dietitian - Ann Hunter is our in-house Dietitian. Dental Hygienist - Kelly Robinson is a licensed R.N. and a licensed Dental Hygienist. |

| Transportation: | ||
| The facility provides transportation for residents to physician, dentist, podiatrist, optometrist, orthopedic, x-ray and all other medical related appointments. | ||
| CATC - Transportation for residents of the facility is also provided by the City of Casper public transportation bus upon request by the residents. | ||
| Activities: | ||
| There are full-time activity coordinators for each of our four units. Activity staff provide daily activities seven days a week. Activity staff also escort residents outside the facility for functions in the Casper community year round and on shopping trips. | ||
| Dining Facilities: | ||
| There are four separate nursing units within our facility. Each unit has its own private dining room with hot and cold food service from its own pantry. | ||
| Convenience Store: | ||
| The La Tienda store is on site for small personal item needs of the residents. Residents may charge to their trust accounts at the store. | ||
| Telephone Service: | ||
| All rooms are wired for a private telephone if the resident wishes to have a private telephone installed. The facility provides access to free local telephone calls at the nursing stations. For residents unable to come to the nursing stations, mobile phones are available. | ||
| Lab Services: | ||
| Every Monday and Thursday a licensed lab technician is in the facility for routine lab draws. Acute lab draws are provided by our licensed nursing staff. | ||
| Pharmacy Services: | ||
| Prescription medications are supplied in single dose blister packs for residents of the facility to assure medication accuracy and safety. | ||
| Resident Trust Accounts: | ||
| The facility has a Resident Trust Account through Hilltop National Bank of Casper. Residents are encouraged to deposit their personal funds in their trust accounts for safe keeping. Accounts of$50 or more are paid the current bank interest rate. Residents may withdraw funds seven days a week at the receptionist’s area. For residents who are on Medicaid and whose checks are sent to the facility, the resident’s $50 monthly personal funds are deposited into their personal trust account. | ||
